I think Fuwawa loves Mococo in a more romantic way but Mococo loves Fuwawa in a more familial way.
It makes sense that they've always been close from not only being sisters but also twins that they are naturally closer than regular siblings, taking into account the various studies on twin relationships.
Now we know that Mococo has sporadic migraines that are bad enough to be disabling, and as the older sister where does that leave Fuwawa when the closest person in the world to her is down and in pain? the care taker role. we've heard that Mococo is "always the little spoon" so my assumption is that this stems from when she has migraines she is taken care for by Fuwawa including food, drink and a warm embrace from behind to help her relax or sleep so that the migraine can pass. now as the caretaker i believe Fuwawa has experienced the Florence Nightingale effect which has lead to romantic feelings toward her little sister which surface as her siscon tendencies, while Mococo as the recipient of her sisters care has a deep feeling of familial love from being nursed and cared for.
Mococo is not blind to her sisters true feelings however and even though she doesn't feel the same romantic feelings as her sister, she does
reciprocate them in small amounts due to her feelings of gratitude.
