But IF this is even possible, then *maybe* it'd still be you, since it would be gradual and you wouldn't lose your consciousness during the process. I imagine that the real (You) is spread throughout your entire brain, so while one piece is removed and replaced it kinda stays alive on the rest, and when the new piece is fully incorporated it can function as part of your natural brain so you can replace the next one.
But that's just what I think. Maybe there's like one piece in your brain that I'm not aware of that would actually mean the death of your real (You) if it was removed even for an instant.
