My best explanation is that it's a place (restaurant or whatever) that needs to have a wheelchair accessible toilet but they didn't measure how much space they needed, so they just hooked up the second toilet too and called it a day. It would explain the lack of stalls too, they probably thought leaving the stalls off would save them a bit more space.
