A streamer's job isn't to play games, it's to entertain the audience. Playing games just gives you a framework in which to do that.
Streamers can also have a bunch of work that you never directly see (or never put a time on), which could be wrangling assets, getting permissions, making thumbnails. In hololive's case this could include singing/dancing/voice training, voice work for hologra, general workplace meetings (which might not take energy, but if you're stressed out about other things, scheduling around meetings can add to it).
The exact amount of work any streamer needs to put in (outside of the streams themselves) is going to vary, but I'd expect hololive vtubers are on the higher end of the scale, vs. someone who just streams on twitch whenever they want without a schedule.
