Rushia is still the only hololive member that ever received an official alternative hair color (and I'm not talking about something like how Gura changes her OBS pallet for Gawr).
But if you want a more detailed explanation for each of those instances you listed, I guess I'm not doing anything right now and I'm kind of bored, so I can explain them. There's a lot of history here.
>Korone - furball, tree, Fubuki - KurokamiKedama and Tree Korone have specific rules about their use and Korone isn't allowed to just do an entire stream with them whenever she wants. Fubuki has said the same. Fubuki, especially has to get approval, whereas Korone can bring them out as a gag if she wants.
>Ollie - Olivia (also Kaela's loli model)Once again, they have to get permission to do entire streams with them. Those aren't like outfits that they can use whenever they feel like it.
>Smols (and other unoffical commissioned 3D models)Those aren't outfits and they were made out of necessity because myth didn't have 3D models even after 2 years due to Covid. Additional ones were made for Councilrys so they could participate in VRchat streams before their 3D debuts. Ame recently said that there won't be any additional smol models for other members because Cover is no longer letting them commission non-official 3D VRchat models. Though since Ame said that, an exception was made for Shiori's Valentines stream. For completeness sake, Bae also has an unofficial 3D loli model she used last year, but I'm unfamiliar with the details surrounding that.

Nene and Flare's original designs had to be changed because of the shit that happened with China. This is also why Flare's mascot, a floating panda fairy named Kintsuba, was permanently retired.
Irys and the homo (and Fubuki) were changed because they requested it for various personal reasons. In Fubuki's case, it was because her artist changed his art style and Fubuki wanted her face to be capable of being more emotive. Irys wanted a model that was simpler and more consistent with anime aesthetics of other members. I don't watch holostars, so I don't know why Vesper wanted his model changed, I just know that he hated it and complained about it like a passive-aggressive bitch on his roommate account by saying that his new job gave him an "ugly uniform" or something.

For Miko though, it's because her original artist (the character designer for Railgun and Index) had some kind of menhera attack and didn't want to be associated with Miko or Hololive anymore. His twitter is still privated years later. For Suisei, it's because she made her original model herself, but Cover wanted her to have a dedicated professional artist as her designer when she transfered to hololive. And then finally, in the case of AZKi, her original designer passed away and so they updated her model when she got a new designer, after that, she had a second redesign by that second artist for when she joined hololive officially. Sora, Miko, and AZKi are exclusively 3D talents and do not have Live2D models, and so getting changes done is even more difficult than other talents.
