For shadows, I prefer the inner shadow. That's enough illusion to make it seem like you have a solid frame. However, if you want to use both, try applying different types of blur to each. Study how shadows are supposed to behave, and adjust opacity and blend mode
No problems with solid colour or texture background.
If you want to use the scaled image bg, you don't need to adjust that layer's opacity. Simply add a new white or black layer on top, set it to overlay, and adjust the opacity of that layer instead. Non-destructive, and you can easily revert if you want to.
