I try not to be cynical, but even the "helping people" motive falls flat in this profession. People are stupid. Can you feel good about yourself and proud of the countless hours of work you've put into learning everything there is to know about the human body, just after you extricate a wine bottle out of someones ass? You will be forced to save people who have no right being saved. And you won't be able to save those who so deserve to be saved. My old ER had a saying that "Scumbags live forever". You'll have to give an ER room to a homeless guy for a night because he learned how to game the system by saying he has chest pain, over and over again.
This is your reward for being the best and the brightest. For devoting thousands of hours of your early 20s to early 30s for this. Not to mention the anxiety, depression, and whatever the fuck else.
