First you need to ask yourself big questions, like what it is that you want or desire for yourself. Don't skimp this part, really take the time to visualize what you want, ask yourself what would your life be like if you could design it any way you want. Then holding that vision, break it down into a traceable path of smart goals. This is crucial, and in fact, it's all discipline really is. Discipline is holding a vision for yourself, and doing one small thing TODAY that gets you closer to it. All it takes is you completing one small thing per day towards a goal you have, and then simply repeating that. You don't have to all figured out either, because as long as you have a sufficient why, the how will naturally unfold. Let's take fitness for example. Hold a vision of what you want your body to look like, then to the best of your ability, make a logical path of small goals to get there, then start anywhere. Try some sort of workout, try to eat clean, something etc. You will fail, you will get discouraged, you'll think you're doing it wrong/that it's pointless, but if you stick with it, and keep doing something DAILY, you'll learn from your failures, you'll adapt, you'll learn the right way to lift/diet, etc, and eventually you'll hit your goal.
